Tyler Green, DPTPhysical Therapist, Co-Founder
Tyler opened Rise Physical Therapy in 2016. He was born and raised in Arkansas. He received his Bachelors of Science in Biology from Arkansas Tech University and his Doctor of Physical Therapy (DPT) from the University of Tennessee Health Science Center. Tyler has two daughters, Katelynn and Hannah. He enjoys spending time outdoors rock climbing, skiing and hiking.
Derek Lee, DPTPhysical Therapist, Co-Founder
Derek opened Rise Physical Therapy with Tyler in 2016. Derek is a Fayetteville native and graduate of the University of Arkansas, where he received his Bachelor of Science in Kinesiology in 2007. He then received his Doctor of Physical Therapy (DPT) at Southwest Baptist University in 2011. After PT school, Derek and his wife Erin moved back to Fayetteville where they continue to put down roots and raise their son, Isaiah. Derek has a passion for running, cycling, and fly fishing.
Alyssa Lindau, DPTPhysical Therapist
Alyssa joined Rise Physical Therapy in September 2017. She is a Denver, Colorado native and received her Bachelor of Science in Rehabilitation Science from Northeastern University in Boston, MA. In May 2017, she graduated from Northeastern with her Doctor of Physical Therapy (DPT). Alyssa enjoys running, biking, hiking and spending time outdoors with her dog. She has run the Boston Marathon and is excited to be able to work with runners and athletes in Fayetteville.
Jeanette Turley, DPTPhysical Therapist
Jeanette joined Rise Physical Therapy in January 2018. She completed her bachelor’s degree at the University of Arkansas and received her Doctor of Physical Therapy (DPT) from UT Southwestern Medical Center in Dallas, Texas. After PT school, Jeanette and her husband Clayton moved back to NWA. They recently took on a fixer upper and spend most weekends performing DIY home repairs when they are not soaking up the outdoors. Jeanette enjoys gardening, running, and spoiling their three dogs.
Carrie Ward, DPTPhysical Therapist
Carrie joined Rise Physical Therapy in July 2018. She's a Springdale native with a Bachelor of Science in Kinesiology and a DPT from UCA. In her spare time, Carrie enjoys traveling, food, sports, exercise and spending time with her dog Sadie.
Jacob Davies, DPT,Physical Therapist, Springdale Clinic Director
Jacob joined Rise Physical Therapy in April 2021. He is a Northwest Arkansas native and attained his Bachelor of Science in Kinesiology in 2015 from the University of Arkansas. He went on to graduate from Southwest Baptist University in 2019 with his Doctor of Physical Therapy (DPT). His special interests include working with athletes of all ages, ACL injury prevention, and global health outreach. He enjoys spending time with his wife, Hannah, their son, Levi, and Australian shepherd, Chip. He loves to get outdoors, explore, and cheer on his Arkansas Razorbacks and Kansas City Chiefs.
Jessie Czarnecki, DPTPhysical Therapist
Jessie joined Rise Physical Therapy in September of 2020. She completed her Bachelor’s degree in Biology and Neuroscience at Augustana College, and received her Doctorate in Physical Therapy (DPT) at Washington University in St. Louis in 2020. She moved to Fayetteville shortly after graduating PT school. Jessie is passionate about coaching and participating in CrossFit, and hopes to grow roots within the NWA community to work with athletes of all backgrounds, with an emphasis on CrossFit. She also has a passion for artwork, running, and hiking in her free time.
Adam Blake, DPTPhysical Therapist
Adam was born in Greenbrier, AR, and attended Harding University in Searcy, AR for his Bachelor’s in exercise science; then graduated from Harding with his Doctor of Physical Therapy in 2021. Adam has his CrossFit Level 1 certification and is passionate about working with lifting athletes and the active population. Adam and his wife Morgan live in Rogers where she is a Labor and Delivery nurse. When not at Rise Adam enjoys hiking, paddle boarding, and CrossFit.
Kelsey Grainger, DPTPhysical Therapist
Kelsey joined the Rise Team in September 2021. She completed her Bachelor’s in Psychology at John Brown University in 2015 and earned her Doctorate in Physical Therapy (DPT) at Washington University in St Louis in 2021. Kelsey enjoys working with athletes of all levels. She also has a special passion for women’s health and has completed additional training to practice pelvic floor physical therapy. In her free time, Kelsey enjoys hiking, running, weightlifting, and rock climbing, spending time with her husband, gardening, and playing acoustic guitar.
Megan James, DPTPhysical Therapist
Megan graduated with her Doctorate in Physical Therapy from the University of Arkansas for Medical Sciences in 2021. Since graduation, Megan has practiced in pelvic floor therapy developing a love and passion for the field. Megan is a member of the APTA section on Pelvic Health and certified in internal and external vaginal exams through APTA. Megan sees patients of all genders and ages. Megan has a passion for treating chronic pelvic pain and pain with intercourse. She is a trauma-informed therapist who recognizes its impact on pain and quality of life. In addition, she specializes in treating urinary and fecal incontinence, pelvic organ prolapse, interstitial cystitis, endometriosis, constipation, diastasis recti, vaginismus, dyspareunia, post-prostatectomy, and postpartum/pregnancy. She is also a dedicated runner favoring marathons and ultramarathons. This sport helped her develop a love for helping women return to running after pregnancy. Megan lives in Fayetteville, AR with her husband, Elliott, and two children, Lola and Mabrey. She is a former theatre kid who started her physical therapy career later in life. In her free time, you can find Megan playing board games at a local brewery, coordinating the Mustache Running Club, reading a dystopian novel, or volunteering at a local race in an inflatable chicken costume.
Kyler Offenbacker, PT, DPT, OCS, CSCSPhysical Therapist
Kyler joined Rise Physical Therapy in April 2022. He is a Springdale native and received his Bachelor of Arts degree from Harding University in 2015. Kyler double majored in Pre-Professional Health Sciences and Leadership & Ministry and played 4 years of collegiate baseball as well. He received his Doctor of Physical Therapy (DPT) from the University of Arkansas for Medical Sciences in 2019 and then completed an Orthopaedic Residency in 2020. Kyler is a board-certified Orthopaedic Clinical Specialist (OCS) in physical therapy and a Certified Strength and Conditioning Specialist (CSCS) through the NSCA. Kyler enjoys sports performance evaluation and training of all athletes, with a special interest in baseball and golf. Kyler and his wife, Emily, have a daughter named Whitley, and a dog named Chester. When not at Rise, he enjoys spending time with family, playing golf, and woodworking.
